2026-2029 Strategic Plan

Community Action Partnership of Hennepin County (CAP-HC), in collaboration with Creation in Common, engaged in a comprehensive planning process to develop our 2026-2029 Strategic Plan. The framework for the plan was informed by an extensive assessment to ensure CAP-HC’s goals are evidence-based, community-informed, and actionable.

The assessment included:

A SWOT analysis
Board, staff, and community stakeholder interviews
A deep-dive review of internal data

Staff leadership have aligned our 2026 work plan with these strategic goals and will use the strategic plan to inform our 2027, 2028, and 2029 work plans to keep us on track for measurable progress toward each goal.
