MNsure Application Assistance

Whether you’re uninsured, underinsured, or just have questions about health insurance, Community Action Partnership of Hennepin County’s (CAP-HC’s) Navigators provide in-person MNsure application assistance so you can access low or no-cost coverage for you and any members of your household.

Eligibility

You’re eligible for CAP-HC’s MNsure Application Assistance if you meet the following requirements:

  1. You live in Hennepin County.
  2. You are uninsured or underinsured.

Members of a federally recognized American Indian tribe can also sign up through any CAP program at any time throughout the year.

How to Submit Your Application

You can submit your completed application in one of the following ways:

In person:

Visit one of CAP-HC’s offices to drop off your application.

We will review your application materials and follow up with you for next steps. If you’re eligible, we will contact you to make an appointment.

Prepare for Your Appointment

Bring the following documentation to your appointment:

  • Social Security number for each person applying
  • Date of birth for everyone in your household
  • Driver’s license, Tribal ID, or another form of identification
  • Your tax forms from the last year you filed (Form 1040EZ or first page of Form 1040)
  • Your two most recent pay stubs
  • Documents for other income (if you have any)
  • Information about any employer-provided health insurance available to each person applying (you may need to request this from your employer)
  • If you have an existing MNsure account, bring your username and password
  • If you are a U.S. non-citizen, bring a green card or other immigration documents
