Rental Assistance Online Application

Thank you for your interest in Community Action Partnership of Hennepin County’s (CAP-HC’s) Rental Assistance Program. Completing an online application will take 15-40 minutes, depending on the size of your household.

The first step for completing your online application is to determine your preliminary program eligibility* by completing the form below. If the information provided indicates your preliminary eligibility, you will be directed to the full online application.

*Preliminary eligibility does not confirm your eligibility or guarantee you’ll receive services from CAP-HC.

Required Documents

To complete the full online application, you’ll need to have these documents ready to upload:

  • A copy of your lease or rental agreement.
  • Proof of the household size such as a lease, current tax return, or benefits statement.
  • Proof of the Hennepin County address, such as a utility bill, current tax return, or benefits statement.

If anyone in your households has received income in the last full calendar month, you will also need to upload:

  • Proof of income for all adults in the household for the last full calendar month.
  • This includes all sources of income such as wages, public benefits, social security, child support, etc.
