We are looking for client stories to spotlight via Community Action Partnership of Hennepin County’s (CAP-HC’s) website, publications, and social media. Please take a few minutes to complete the form below.

By submitting this form and giving CAP-HC permission to share your first name, story, and photo(s), you may be eligible for a $25 gift card* as a thank you for your time. If you are eligible, we will arrange a convenient time for you to pick up the gift card shortly after receiving your form and photo(s).
