Vehicle Repair Online Application

Thank you for your interest in Community Action Partnership of Hennepin County’s (CAP-HC’s) Vehicle Repair Program. Completing an online application will take 15-40 minutes, depending on the size of your household.

The first step for completing your online application is to determine your preliminary program eligibility* by completing the form below. If the information provided indicates your preliminary eligibility, you will be directed to the full online application.

*Preliminary eligibility does not confirm your eligibility or guarantee you’ll receive services from CAP-HC.

To complete the full online application, you’ll need to upload:

  • A copy of your valid Minnesota driver’s license.
  • Proof of current auto insurance for the vehicle in need of repair.
  • Proof of vehicle ownership, such as title or tab renewal receipt.
  • Proof of the household size such as a lease, current tax return, or benefits statement.
  • Proof of the Hennepin County address, such as a utility bill, current tax return, or benefits statement.
  • Financial Wellness training certificate(s) showing that you have completed the training within the last 12 months.

If anyone in your households has received income in the last 30 days, you will also need to upload:

  • Proof of the last 30 days of income for all adults in the household.
  • This includes all sources of income such as wages, public benefits, social security, child support, etc.
